England & Wales – Baby Loss Certificate
Since February 2024, parents whose baby was born before 24 weeks (or before 28 weeks if before October 1992) can request a free Baby Loss Certificate from the government.
This certificate has no legal status, but it can be a deeply meaningful way to record your baby’s existence and your love for them.
Who can apply?
Anyone whose baby was born before 24 weeks.
It doesn’t matter how long ago your loss happened.
You can apply even if you don’t have all your hospital records (you may be asked for extra information).

Scotland – Memorial Book and Commemorative Certificate
In Scotland, you can request to have your baby’s details recorded in the official Memorial Book of Pregnancy and Baby Loss Prior to 24 Weeks.
You will also receive a commemorative certificate, free of charge.
Who can apply?
Anyone whose baby was born before 24 weeks in Scotland.
There is no time limit, and no medical evidence is required.
